The Pakistani government has introduced the Child Registration Certificate, commonly known as the B-Form, for individuals under the age of 18.
This ID document is a fundamental right for children, registering them in government records and serving as a requirement for school admission and various other purposes.
The National Database and Registration Authority (NADRA) oversees the process of acquiring the B-Form for all Pakistani children, ensuring access to basic services. In recent years, the government has implemented measures to streamline the issuance of these documents for the convenience of the masses.
The B-Form is issued from the child’s place of origin, and obtaining it requires providing proof of childbirth from the union council.
NADRA B-Form Fee
As of December 2023, the standard fee for applying for the B-Form is Rs 50, with NADRA also offering executive services for added convenience.

- Valid ID cards of both parents
- Proof of Birth: Nikkah Naama (Marriage Proof)
Application Process for B-Form in Pakistan
- Visit the nearest NADRA office and obtain a token for the B-Form.
- Undergo the photograph procedure.
- The operator will capture fingerprints and the applicant’s signature.
- Submit the required data and receive the printed form.
- Obtain the printed version of your application form.
- Submit the form to the relevant NRC for processing.
