The B-Form, or Child Registration Certificate, is a primary ID that verifies an individual’s birth details, identity, age, and record in NADRA. Parents should obtain a B-Form as it is essential for acquiring other documents and accessing basic services like driving licenses, passports, and school or college admissions.

The CRC or B-Form includes the child’s name, registration number, date of birth, parent ID, and address, making it crucial for establishing identity. The National Database Registration Authority (NADRA) is responsible for issuing the Child Registration Certificate or B-Form for all minors.

To obtain a B-Form, parents must provide documented proof of childbirth from the union council. The parent must also hold a National Identity Card (NIC) or National Identity Card for Overseas Pakistanis (NICOP).
